Admission

If you would like to become a resident a Fifth Season Residential you must take the following steps:

  1. Make an appointment with management to undergo an interview.  The interview will take approximately one hour and will consist of a "Prospective Resident Screening", and a discussion to determine services needed, options available, and cost*.
  2. You must fill out the application form in its entirety to submit during the meeting.  A list of medications must be submitted during the meeting also.
  3. Undergo a "Physician Certification" physical by your family doctor and submit the Physician Certification form to us before admission to the facility.
  4. Once your application and screening have been approved, you must submit a $250.00 damage deposit in order to hold an apartment.  The deposit can be held for up to 2 weeks.  The deposit is refundable upon termination of agreement**

Section 295.2030 (a)(7) of the Assisted Living and Shared Housing Establishment Code requires that the assisted living establishment provide the resident, a base rate, and a description to be provided.

Rates:

Fifth Season Residential, LLC charges a "flat rate" for all housing services offered.  The flat rate, referred to herein as the "base rate" is the monthly amount charged to each resident while a resident, and will differ depending on the size of apartment, location of apartlment, number of bedrooms within each apartment, the apartment layout, and the number of people sharing the apartment.  The individual monthly base rate, per resident, for each apartment ranges from $1850.00 to $3000.00.  If two related people share a one-bedroom apartment or a studio apartment, then the charge for the second (related) person is $850.00.   This fee is added to the full base rate.  No more than two people can reside in an apartment at any time.

 

Deposit:

All new residents must give a damage deposit of $250.00 in order to hold an apartment.

Once the deposit is given, Fifth Season will hold the apartment for a two-week period of time.  Should the resident desire to continue to hold the apartment past the two-week time period, the resident must pay rent, beginning the date following the two-week holding time.

The deposit is retained until each resident moves out.  The deposit is then refunded, less the amount used for repairs.

Monthly Rent:

A monthly statement will be mailed to each responsible payer before the end of each month. Each resident't rent is due in full on the first day of each month.
